A Scripps Howard News Service report finds that states are doing a wildly varying job when it comes to identifying the source of food and water borne illnesses:
Scripps studied 6,374 food-related disease outbreaks reported by every state to the federal Centers for Disease Control and Prevention from Jan. 1, 2000, through Dec. 31, 2004. The causes of nearly two-thirds of the outbreaks in that period were officially listed as “unknown.”…
The poor track record of so many state labs also raises chilling questions about their ability to spot or deal with a food-borne terrorist attack….
The study found that Kentucky, Oklahoma and Nebraska are virtually blind to outbreaks of food sickness, rarely detecting that scattered illnesses have common food causes.
In Alabama, Florida and New Jersey, the cause of food poisoning is almost never found, even when it is known that dozens or hundreds of people became violently ill or died from something they ate, according to the Scripps study….
Alabama was the worst in the nation, diagnosing only 5 percent of its reported outbreaks, the study found. “It’s a real struggle. We’ve never identified a virus at the state level,” said Alabama State Epidemiologist John Lofgren.
After learning of the study’s findings, Kentucky officials ordered changes to their disease-reporting system. “We really hadn’t been categorizing food- and waterborne outbreaks,” admitted Kentucky Epidemiologist Kraig Humbaugh.
During the five-year period studied, Florida reported only seven people sickened by E. coli outbreaks, a suspiciously low number for a state of its size. Nationwide, at least 3,349 people contracted E. coli in food-poisoning outbreaks….
The study found that health departments are more likely to make a diagnosis when a very large number of people get sick. They failed to determine the cause in 31 percent of the outbreaks that sickened 50 people or more. But the failure rate increases rapidly with smaller groups.
Fifty-three percent of outbreaks affecting 10 to 49 people went undiagnosed, while 75 percent of outbreaks that sickened nine or fewer people were listed as “unknown” causes.
Several state and local epidemiologists said large outbreaks give them more chances to isolate the exact disease involved. More victims mean a better chance of obtaining blood, stool and urine samples that can be tested for specific pathogens.
But epidemiologists admit that failures to diagnose food illness are common, even when the only suspect for outbreaks of a widespread intestinal disease is food. The Scripps study found that the disease went undiagnosed in 4,054 of the 6,374 reported outbreaks. Those unknown causes sickened or killed 50,968 people….
Every year, an estimated 5,000 Americans die from food-based diseases like Salmonella, E. coli, Shigellosis and Campylobacter. Another 325,000 people are hospitalized. The CDC estimates that food-based sickness probably afflicts 76 million Americans annually.
Although the Scripps study found that the quality of the nation’s network of public health departments varies alarmingly, there were some bright spots.
Wisconsin, Minnesota and Hawaii do a good job of diagnosing disease outbreaks.
Wisconsin came out on top in the study by diagnosing the cause of 90 percent of its food-poisoning cases. Wisconsin also was the first state to detect and report September’s deadly E. coli outbreak from infected raw spinach grown in California and shipped nationwide. The outbreak killed at least three people and sickened at least 199 others.
But the study found little to celebrate overall since most outbreaks go undiagnosed.
Federal officials and public health experts agreed with the findings and conclusions of the Scripps study.
“Our surveillance systems were designed to ring a bell when there is a problem. Are they perfect? Absolutely not. Could they be better? Absolutely yes,” said spokesman Tom Skinner at the CDC’s Atlanta headquarters after reviewing some of the study’s findings. “We’ve already come a long way, but certainly, we can do better than this.”
Skinner offered no explanation when asked why the CDC didn’t warn underperforming states and local health departments.
“The CDC, like most government agencies, is pretty conservative. Why would they want to rock the boat?” said Ewen Todd, director of the Food Safety Policy Center at Michigan State University. “It takes someone who is independent to say: ‘This is crazy’.”
Todd agreed that the quality of public health is erratic in the United States. He said state health programs are especially poor in the South.
“Our laboratories are pretty good. But, overall, the whole public health system is not working very well,” Todd said. “There are no national standards for the surveillance and reporting of food illnesses.”
Other experts said they believe the quality of state labs also varies alarmingly.
“Lab capacity is a serious issue for many reasons,” said Jeffrey Levi, executive director of the Trust for America’s Health in Washington, which studies trends in public health. “The federal government should set minimum standards and expectations and provide the resources necessary to assure this capacity.”
The report also explains why small, local producers often seem to have better food safety records. As we’ve harped on in the Pundit, these small producers cause many small outbreaks, virtually none of which get linked to a specific producer. As the Scripps Howard piece states:
“Several state and local epidemiologists said large outbreaks give them more chances to isolate the exact disease involved. More victims mean a better chance of obtaining blood, stool and urine samples that can be tested for specific pathogens.”
So based on this statistical chimera, newspapers and magazines across the country are urging local production to displace California, Arizona, Washington, Texas, Florida and other major national shippers.
The implication of this study is that now that attention is being paid to this matter, state labs will be upgraded and under-reporting states such as Florida will identify many more outbreaks of E. coli 01257:H7. This means that we have to get constantly safer to keep the number of outbreaks constant.
Like gerbils on the wheel.

The big project right now is preparing to launch the Camino Màgico Program nationwide:
Our Camino Màgico Supermarket Guide is being designed, borrowing inspiration from the graphic arts of Latin America and the still lives of masters such as Fernando Botero, Frida Kahlo and Diego Rivera. Our classic Latin American Diet Pyramid is also receiving a face-lift as we include graphics depicting activities such as dance, soccer, walking and aerobics at the bottom of the Pyramid, and work upwards with new graphics to better echo Latino culture.
Papayas, mangos, pineapples and limes will be included in the fruit section, with a new array of colorful choices filling the space in our vegetable section. Other changes will be made and unveiled with the first edition of the guide, which resembles a 16-page passport that will open doors to a healthier life.
To launch the Camino Màgico program, we are targeting Miami, Houston, Los Angeles, New York, Chicago and Boston. Our launches will feature local celebrities, nutritionists and scientists, and will be held at sponsoring supermarkets in these target cities. After an initial one-hour symposium, we plan to lead the media on a healthy supermarket tour. Each event will culminate in cooking demonstrations with plenty of food and innovative physical activities.
In preparation for these launches, we are creating distribution alliances with supermarkets, churches and health centers in our target cities, expecting at least one million supermarket guides to be in circulation during 2007. The guides, to be printed three times in 2007, will include dishes ranging from the traditional Arroz con Pollo to a more elaborate Pescado Veracruz, including a total of twelve meals and various snacking suggestions. We encourage everyone interested in the health of Latinos to join us!
We are glad to see a program that might actually work and certainly won’t insult anyone. We like the inclusion of physical activities, fresh vegetables and papayas, mangos, pineapples and limes.
It has always frustrated the Pundit that mangos, with their worldwide popularity, don’t get the retail display space and price points that the fruit merits. Bet an outreach to Latinos will wind up introducing plenty of Anglos to this fantastic fruit.

As we go off to the holidays, this Pundit just wanted to say THANK YOU for allowing us to be part of your daily lives. Since we launched our first page on August 7 of this year, hundreds and hundreds of e-mails and letters, even more phone calls, have poured in.
The Pundit was born to the industry and has been part of it as an adult for over 20 years, but the intimacy of a daily, the interconnectivity of e-mail and the internet means that we feel like we have friends we never had before. So we get to wish a fantastic holiday season and a wonderful 2007 to so many people at once.
If you celebrate Christmas, and even if you don’t, it is a great American literary tradition to read to the children “Twas the Night Before Christmas” each Christmas Eve. It was first published anonymously in the Sentinel, a newspaper in upstate New York, on December 23, 1823, and was an instant success.
Two decades later Clement Clarke Moore acknowledged authorship. The author’s father, Benjamin Moore, was President of what is now Columbia University and the Bishop of New York. He officiated at the swearing in of George Washington as the first President of the United States and gave last rites to Alexander Hamilton as he lay dying after a duel with Aaron Burr.
Though Clement Clarke Moore was a noted scholar of Hebrew, was conversant in five languages and an accomplished real estate developer, he is remembered only because he gave us this gift and, in doing so, defined for generations how we imagine Santa Claus. Which shows that words have a power that is hard to overstate.
Here at the Pundit, we try hard to be worthy of that responsibility.

Thought a great thing to do with our last Pundit’s mailbag of 2006 is feature our first letter from outside North America. It is in response to our piece analyzing the concept of “Fairtrade” in the context of J. Sainsbury’s commitment to “Fairtrade” bananas:
The Pundit always listens to Marc’s suggestions. Maybe it is because of his experience on the PMA board, maybe because he has great pictures, mostly though it is because we would never cross his mother-in-law, Claire Thornton, who, among many other things, hosted Fresh Ideas, a television show on the Family Channel sponsored by Dole and Sunkist.
Seriously, Marc has a lively intellect and, in pointing us to the noted British publication (it calls itself a newspaper but looks like a magazine), he is finding a view not unsympathetic to the Pundit’s. As the article states:
Fairtrade food is designed to raise poor farmers’ incomes. It is sold at a higher price than ordinary food, with a subsidy passed back to the farmer. But prices of agricultural commodities are low because of overproduction. By propping up the price, the Fairtrade system encourages farmers to produce more of these commodities rather than diversifying into other crops and so depresses prices — thus achieving, for most farmers, exactly the opposite of what the initiative is intended to do.
And since only a small fraction of the mark-up on Fairtrade foods actually goes to the farmer — most goes to the retailer — the system gives rich consumers an inflated impression of their largesse and makes alleviating poverty seem too easy.
What we didn’t deal with but The Economist does is other shibboleths of modern society. Thinking of going organic to be environmentally friendly:
Organic food, which is grown without man-made pesticides and fertilisers, is generally assumed to be more environmentally friendly than conventional intensive farming, which is heavily reliant on chemical inputs. But it all depends what you mean by “environmentally friendly”. Farming is inherently bad for the environment: since humans took it up around 11,000 years ago, the result has been deforestation on a massive scale.
But following the “green revolution” of the 1960s, greater use of chemical fertiliser has tripled grain yields with very little increase in the area of land under cultivation. Organic methods, which rely on crop rotation, manure and compost in place of fertiliser, are far less intensive. So producing the world’s current agricultural output organically would require several times as much land as is currently cultivated. There wouldn’t be much room left for the rainforest.
Or maybe you are just going to buy local food at farmer’s markets to minimize environmental problems due to lengthy transport:
Surely the case for local food, produced as close as possible to the consumer in order to minimise “food miles” and, by extension, carbon emissions, is clear? Surprisingly, it is not. A study of Britain’s food system found that nearly half of food-vehicle miles (i.e., miles travelled by vehicles carrying food) were driven by cars going to and from the shops. Most people live closer to a supermarket than a farmer’s market, so more local food could mean more food-vehicle miles. Moving food around in big, carefully packed lorries, as supermarkets do, may in fact be the most efficient way to transport the stuff.
What’s more, once the energy used in production as well as transport is taken into account, local food may turn out to be even less green. Producing lamb in New Zealand and shipping it to Britain uses less energy than producing British lamb, because farming in New Zealand is less energy-intensive.
Besides, these programs often contradict one another:
And the local-food movement’s aims, of course, contradict those of the Fairtrade movement, by discouraging rich-country consumers from buying poor-country produce. But since the local-food movement looks suspiciously like old-fashioned protectionism masquerading as concern for the environment, helping poor countries is presumably not the point.
At holiday time when charity is on the mind, it is worth remembering that many things that make people feel good are not actually doing any good.

There is an ad-hoc group that started it all, the National Restaurant Association has its group working on a program and the Food Marketing Institute held a conference. All these buyer-led initiatives can get confusing, so to assist the trade in keeping track of them all, we are publishing this recap of coverage all in one place.
As new developments, occur we will continue to update this recap to help keep the trade organized on this important subject.
On September 25, 2006, in the midst of the spinach crisis, we published The Role of Retailers And The Future Of Food Safety, which pointed out that it is the “representations and warranties” that buyers demand that define the food safety programs we get:
“…in the end, the strength of our food safety systems is at least as dependent on what retailers demand as they are on what the government does for the simple reason that what retailers pay for is what they are going to get.”
Then in the issue of the Pundit’s sister publication, PRODUCE BUSINESS, which was unveiled at the PMA Convention in San Diego on October 21, 2006, we published Food Safety Is A Retail Issue, which pointed out:
“…what holds suppliers back is not that they need an FDA regulation — it is that they need to see a willingness on the part of buyers to pay more to obtain a higher level of food safety and security. So far that is missing.”
The Buyer-led Initiative for Food Safety was then announced. In time it came to be signed on to by nine important buying organizations:

Here at the Pundit, we applauded the buyer-led effort but on October 30, 2006, ran a piece entitled Buyer-Led Food Safety Effort Leaves Open Question Of Buyer Commitment, in which we pointed out:
“What would be helpful from these buyers is…a reassurance to the grower/shipper/packer/processor community that investments in food safety will be protected.”
As Gene Harris of Denny’s added his endorsement to the Buyer-led Initiative for Food Safety, we published, Pundit’s Mailbag — Denny’s Weighs In On Food Safety Efforton November 1, 2006, and we pointed out that the Western Growers Association was now looking for mandatory standards:
“Buyers can impose standards on their suppliers, but it seems as if the big grower members of WGA are more inclined to go with a mandatory program. Perhaps because this is more easily “saleable” to consumers, perhaps because the growers have no confidence that buyers will ever agree to a uniform standard on food safety and, perhaps, because growers know that buyers today can have the best of intentions but situations change and buyer’s change — and if legal product is available for much less money, that will put a lot of pressure on an organization to change its standards.”
On November 2, 2006, we highlighted an Opportunity For Buyers’ Food Safety Initiative, where we wrote the following:
“Here’s the Pundit’s suggestion to the buyers: Don’t wait for the deadline to pass. Withdraw the letter to the associations, which can only lead to endless negotiations with grower/shippers and watered-down food safety standards. Instead, create a temporary ad hoc consortium to spearhead the quick development of science-based food safety standards.
In the short term, these will be enforced by buyer demand, hopefully including other buyers who will buy into the plan; in the medium run the plan will be turned over to state authorities in California and federal authorities in Washington, D.C., as the basis for new mandatory regulation.”
We pointed out that this initiative may not stay in the hands of the ad hoc group leading the Buyer-led Food Safety Initiative when, on November 7, 2006, we announced: National Restaurant Association Forms Produce Safety Working Groupand pointed out:
“What we should have learned from the FDA loss of confidence in the industry is that food safety is not something that we negotiate over. It has to be driven by the best scientific knowledge we have.”
Mark Munger of Andrew-Williamson Fresh Produce, a grower/shipper, pitched in his thoughts on the important role buyers play in the food safety arena and, on November 8, 2006, we published Pundit’s Mailbag — Insights From A Conscientious Grower, which specifically praised one foodservice customer:
I also have to commend one of our customers, who I believe demonstrates the value of collective partnerships between growers and customers. Two years ago we began working with Darden Restaurants. Darden takes food safety very seriously. They have empowered a food safety team that must approve each and every supplier. They have inspectors in the field who make weekly random inspections of growing operations, picking and packing programs. When problem issues are identified, they work closely with our food safety team to help educate our team and to ensure that collectively we fix the problem. The knowledge that an inspector can be in any field or packing shed at anytime has forced us to treat every day as an inspection day.
Additionally, Darden’s food safety team is separate from their buying team. If a farm is not up to par, they have the authority to stop all transactions until the problems are fixed. They truly put their money where their mouth is and have helped us become a markedly better company. I cannot think of a better example of the power of collective thinking between suppliers and customers. I think the industry would be well served to learn more about their programs and create similar models.
Not surprisingly, the Food Marketing Institute was not going to be content to sit this one out and, on November 10, 2006, we published FMI Steps Into The Food Safety Fray, which detailed a conference scheduled for December 5th at which FMI would host representatives from industry, associations, academia and government to advance food safety issues. Unfortunately, FMI decided to exclude the media and we pointed out:
“…if the goal is to build public confidence in the process the industry is going through, you not only open it to media, you send a velvet invitation to the big consumer media groups.
It smells of smoke-filled rooms where deals will be cut in secret. If you let in some light and air, everyone will have more confidence in the final product.”
On November 14, 2006, we published Pundit’s Mailbag: Grower/Shipper Calls Buyer Led-Food Safety Initiative Hollow Call To Action, in which a respected grower/shipper pointed out that “This is where the retailers must step out of their ivory towers and get their walk (vendor relationship) to match their talk (aligned supply chain)… If those who signed on to this letter would get committed to buying only from “qualified suppliers,” the laws of supply and demand will drive the solution and we will quickly catch up with the rest of the world in this critical area.”
On November 17, 2006, we featured Tale Of Two Buyers, in which we pointed out: “If the VPs are sincere about wanting the buyers to place food safety first, the VPs have the responsibility for changing the culture and the economic incentive systems.”
On November 21, 2006, we published Tim York Takes Leadership Role In Food Safety Crisis, which features an extensive interview with Tim York of Markon Cooperative as well as the announcement that the Buyer-led Food Safety Initiative gained ten new retail signatories:

Despite the impressive show of buyer support, we expressed some concern: “…it is also pretty clear that the prospect of one unified food safety standard acceptable to every one of the signatories, much less to those who have declined to sign, is somewhere between nil and nothing.”
On November 28, 2006, we published Words From Buyers Who Did Not Sign The Food Safety Initiative, and in this piece we added Mark Hilton, Vice President of Produce and Floral for Harris-Teeter, based in Matthews, North Carolina, as a signatory to the letter.
We also quoted buyers who had declined to sign the letter mostly due to their objection to the public nature of the initiative. We also pointed out how vendors were thinking:
Pundit Note: Many growers and shippers are irate over the effort as they see it as an evasion of responsibility. These buying organizations get exactly what they value enough to pay for. All too often, some of the same companies who signed the letter on Monday will, on Tuesday, buy some product without the slightest knowledge of where it came from.
On November 29, 2006, we ran Another Naysayer of Buyer-led Food Safety Initiative, which gave voice to the thoughts of some non-participating buyers that only mandatory government regulation is the way to go. On November 30, 2006, we continued our exploration of why some buyers were declining to join the buyer-led initiative with Self-Interests Play Role In Food Safety Initiatives. Also on November 30, 2006, we received a letter from Al Zuckerman of ProMark Group, which we focused on in Pundit’s Mailbag — Pundit Logic On Food Safety Regulation. We pointed out: “In terms of the difficulties on spinach and leafy greens, the key buyers are missing from the Buyer-led Food Safety Initiative. The buyers of the produce, in this case, are the processors.”
On December 1, 2006, we published Spinach And The Consequences Of Buyers’ Action, in which buyers who hadn’t signed on to the buyer-led food safety initiative pointed out that rigorous food safety systems will restrict supply and raise prices.
As we explained: “It is unknown if those who don’t buy spinach because of high prices will buy healthy alternatives. They may buy candy bars and die of complications of obesity. It is a completely open question as to whether safer spinach won’t cost lives in the end.”
Also on December 1, 2006, we responded to industry feedback claiming that foodservice did a better job than retail when it came to food safety by beginning a series of Pundit Pulses focused on foodservice.
